What is Unite for HER?
Unite for HER is a national nonprofit organization that women, men, and nonbinary individuals affected by breast and ovarian cancers. Unite for HER’s free Wellness Programs educate patients about integrative cancer care and provide them with access to supportive services and resources at no personal cost. Unite for HER was founded in 2009 by Sue Weldon following her personal experience with breast cancer.
The name ‘Unite for HER’ features an acronym of Helping to Empower and Restore and signifies UFH’s commitment to uniting communities and the medical and wellness fields to support the ongoing needs of breast and ovarian cancer patients nationwide.
Unite for HER is a registered 501(c)3 with EIN 26-4444438. A copy of Unite for HER’s official registration and financial information may be obtained from the PA Department of State by calling toll free, within PA, 800-732-0999. Registration does not imply endorsement.
How do I apply for the Wellness Program?
Unite for HER is proud to offer wellness programs for individuals who have experienced breast or ovarian cancer and are seeking support resources. To apply, visit www.uniteforher.org/apply and complete the form. All applications are reviewed in the order in which they are received. All applicants will receive email communications within 2 weeks of applying, notifying them of their application status and program details. We look forward to supporting you!

What are the eligibility criteria to be considered for the wellness program?
Unite for HER offers two programs free to those affected by breast or ovarian cancer:
- The Wellness Passport Program is for those diagnosed with breast or ovarian cancer, Stages 1-4, within the past 18 months.
- The Empowered Living Program for those diagnosed with breast or ovarian cancer, all stages, regardless of diagnosis date, including those diagnosed with Stage 0/DCIS breast cancer.
Our Wellness Passport Program provides access to funded resources. Please note that due to the high volume of applications we receive, availability is determined by our budget at the time your application is reviewed.
*Applicants are not required to submit medical records, insurance details, income information, or other documentation when applying.
Where is the Unite for HER Wellness program available?
Unite for HER’s Wellness Program is available nationwide, providing meaningful support to those affected by breast and ovarian cancer across the country.

What services are offered through the Empowered Living Program?
Through Unite for HER’s Empowered Living Program, you will learn about the benefits of integrative therapies and how they can support you, gaining access to our supportive community.

How do I access the Member Portal?
Wellness Passport Program members can access their Member Account from our homepage, or go to www.my.uniteforher.org. Please choose “Member Login”.
*Empowered Living Program members do not need to log in to the Member Portal to access services. If you receive a message stating that your account is not yet active, please contact us for assistance at 610-883-1177.
How do I access the Empowered Living Program services?
No login is required to access the Empowered Living Program services. Simply visit the Empowered Living Program website: https://uniteforher.org/empoweredliving/ for a list of available services.
How do I redeem Passport Squares?
Wellness Passport Program members can use their passport squares to personalize their wellness plan. Squares can be redeemed for any combination of services listed inside the passport, allowing you to create a plan that best supports your individual needs. Your passport is valid for one year from the date you receive it. Refer to your Passport for the number of squares needed to redeem each service. To sign up for a nutrition service or to view the provider directory and schedule an integrative therapy session, please log in to your Member Account. You can also schedule a Wellness Consultation for personalized assistance. This is a great option for members who may not be comfortable using a computer or don’t have access to one. We’re happy to offer this alternative to ensure everyone can get the support they need.
How do I check my Passport squares balance?
Wellness Passport Program members can log in to their Member Account to view their current passport squares balance and transaction history. Please note: There may be a delay in updates for in-person services, as our system reflects changes once invoices are received and processed. You can also call us at 610-883-1177 for assistance.
Does my Passport expire?
Yes! Your passport is valid for one year from the date you receive it. You can find the expiration date on the lower left page of the passport or by logging in to your member account. We are unable to extend the expiration date of Wellness Passports.
What happens when my Passport expires?
We are honored to be able to provide ongoing support to our members even after their passport expires. At that time, you’ll transition into our Empowered Living Program, where you’ll have access to all the services it offers. One can still take advantage of unlimited 1:1 Nutrition Consultations, Cooking Webinars, the video library, and all our monthly educational series! You can find a list of our All Access Pass services on the back of your passport.
How do I register for a Wellness Orientation?
Members are welcome to join a live Wellness Orientation to learn how to access available services and explore the benefits of integrative therapies.
To register for a Wellness Orientation, check your Welcome email sent around the time your Care Box arrived for upcoming dates and registration links.
If you need help or can’t locate the email, feel free to reach out to the Wellness Team for assistance at 610-883-1177, prompt 1.
How do I schedule a Wellness Consultation?
Members can schedule a 15-minute phone consultation with our Wellness Team to get personalized support, ask questions, receive help with scheduling services, and learn more about our program one-on-one. Schedule online at: www.uniteforher.org/schedule
How do I schedule a Nutrition Consultation?
Members can schedule a free 30-minute nutrition consultation with one of our 4 Registered Dietitians who specialize in cancer care. During a nutrition consultation, you’ll receive personalized nutrition guidance tailored to your needs—whether you’re in treatment, recovery, or survivorship. How do I schedule an appointment for an integrative therapy session?
Wellness Passport Program members can redeem their passport for integrative therapy services by selecting a provider from the Directory of Providers and contacting the provider directly to schedule an appointment. Unite for HER partners with professionals who have been thoroughly vetted by our team of medical and wellness advisors and approved to provide therapies and services through the Unite for HER Wellness Passport Program. After your service, providers will bill Unite for HER directly. There is no cost to use your passport. You are also not required to tip. Unite for HER has taken care of that for you. Each integrative therapy appointment is redeemable for one square on your passport, unless otherwise specified. Please bring your passport to your in-person appointment.
How do I access the Unite for HER Provider Directory? How do I find Unite for HER providers?
You can access the Unite for HER Provider Directory by visiting www.uniteforher.org/providers.
Wellness Passport Program members can also log in to their Member Account to view the full directory.
How do I sign up for a grocery gift card?
Wellness Passport Program members can redeem 1 passport square for a $75 grocery gift card. Members are eligible to receive a gift card as frequently as once per month. An appointment is required for each gift card requested. If no dates or times are available, please check back for cancellations. The calendar is open for scheduling 1 week before the start of each month.
To request a grocery gift card, log in to your Member Account, click on “Grocery Gift Card,” and choose a date and time that works for you. A member of our Nutrition Team will call you at your scheduled time to complete the order.
How do I sign up for Sunbasket meal delivery service?
Wellness Passport Program members can redeem 1 passport square to receive one delivery of fresh, easy-to-prepare meal kits from Sunbasket. Each order includes 3 meals, and the weekly menu is preselected by our Nutrition Team. A new menu is available every Monday.
To place an order:
- Log in to your Member Account
- Click on “Sunbasket Orders”
- Select “Place a New Order”
- Choose a time frame from the Sunbasket Option drop-down menu
- View the weekly menu
- Click “Submit” to complete your order
How do I sign up for the vegetable farm share program?
Wellness Passport Program members can redeem passport squares to receive a weekly share of fresh vegetables. Delivery and pick-up options vary depending on your location. To sign up for a vegetable farm share program, log in to your Member Account.
Lancaster Farm Fresh has convenient pickup locations throughout the southeastern Pennsylvania metropolitan area, making it accessible to local program members. You do not need to live near Lancaster, PA, to participate in the Lancaster Farm Fresh vegetable share.
Farmbox RX is a vegetable home delivery program available to members of our National Community—those living outside the Philadelphia tri-state area—as well as local members who may not live near a Lancaster Farm Fresh CSA site, lack transportation, or do not have someone to pick up their vegetable boxes.
Please note: The vegetable share does not include fruit, and members are not able to choose which vegetables they receive. Each box is thoughtfully curated by the farm based on seasonal availability.
How do I sign up for a fitness membership?
Wellness Passport Program members can redeem 3 passport squares for a 6-month fitness membership to the Stronger Thru Cancer online fitness program. Alternatively, members may use their squares for a membership to a local fitness club with a completed physician referral form.
*Please note: Local, on-site fitness memberships are only available in select areas of Pennsylvania, New Jersey, and Delaware. To sign up for Stronger Thru Cancer or to view a list of partnering fitness centers and access Physician Referral Forms, go to www.uniteforher.org/fitness.
How do I sign up for a cooking class?
Our virtual cooking classes are a fun and interactive way to connect with our dynamic Nutrition Team, explore new recipes for all skill levels, and discover how nutrition can support you during treatment and beyond. Each class is 60 minutes on Zoom and free to attend (you do not have to use any squares to participate). To register for an upcoming cooking class, go to www.uniteforher.org/foorforthought/.
How do I sign up for a monthly educational seminar or workshop?
UFH Connect is a digital network that features a variety of live and recorded programming. As part of our mission to enrich the health and well-being of those diagnosed with breast and ovarian cancers for life, this digital platform hosts 40 virtual events that educate and inspire our community. How do I access the HER Video Library?

Students Unite for HER empowers students to make a meaningful impact in their communities while gaining valuable experience in leadership, communication, project management, and teamwork. Unite for HER hosts four signature fundraising events each year (Fall 5K Run/2K Walk, Harvest Fall Chef Tasting Event, Pink Invitational Gymnastics Fundraising Meet, and Bloom Spring Fashion Show) to support our mission. 100% of proceeds directly benefit individuals affected by breast and ovarian cancer nationwide through our Wellness Program. Does a patient have to be referred by a partnering hospital?
No. Patients can be referred by hospitals, nonprofit organizations, support groups, healthcare providers, family members, or friends. We welcome referrals from a variety of sources to ensure more individuals can access our support.
Can I nominate a friend or family member for the program?
Absolutely! We encourage you to share our program with anyone diagnosed with breast or ovarian cancer. You can nominate them and direct them to apply at: www.uniteforher.org/apply
Does Unite for HER offer direct financial assistance?
While Unite for HER does not provide direct financial assistance, we’re proud to partner with organizations that do. How is Unite for HER funded?
