Appointment Form Page #
Radius Booking provides a shortcode to embed the booking form on any WordPress page or post — no page builder required. This is the simplest method and works with any theme or editor.
Appointment Form Shortcode

- Create a New Page: Go to Pages → Add Page in the WordPress admin.
- Set the Page Title: Enter a title (e.g., ‘Booking Form’) at the top of the editor.
- Add a Shortcode Block: Click the + icon, search for ‘short’, and click the Shortcode block.
- Enter the Shortcode: Type or paste [radius_booking_form] into the shortcode field.
- Publish: Click Publish to make the booking form live on your site.
