The Form Customization page lets you fully control the look and feel of your booking form.
Navigate to Radius Booking → Settings → Form Customization.
Changes are reflected instantly in the Live Preview panel on the right. The page is divided into four tabs: Design, Labels & Text, Buttons, and Feature Toggles.
1. Design #
The Design tab controls the visual appearance of your booking form, including colors and sidebar imagery

1.1 Colors #
Customize the full color palette of your booking form to match your brand.
- Primary Color: The main accent color used for highlights, active steps, and key UI elements.
- Secondary Color: A supporting color used for backgrounds and subtle accents.
- Background Color: The main background color of the booking form.
- Text Color: The color used for all body text on the form.
- Sidebar Color: The color of the left-side summary sidebar. Leave empty to use the Primary Color (e.g., #000000).
- Step Button Color: The color of the step navigation buttons.
- Step Button Hover Color: The color displayed when a customer hovers over a step button.
- Border Color: The color of form field borders and dividers.
1.2 Sidebar Illustration #
Upload a custom image to display in the booking form’s sidebar panel, replacing the default illustration.
- Custom Image: Click Upload Image to add your own graphic. Recommended size is 200×200px. PNG or SVG with a transparent background works best.
2. Labels & Text #
The Labels & Text tab lets you customize all the text displayed on each step of the booking form — headings, field labels, placeholders, and status messages. Changes appear instantly in the Live Preview panel.

2.1 Services Step #
Customize the text shown on the first step of the booking form where customers select their service.
- Heading: The main title displayed at the top of the Services step (e.g., ‘Easily book your appointments & Get the Service you deserve.’).
- Service Label: The label shown above the service dropdown (e.g., ‘Which Service you want to take’).
- Location Label: The label shown above the location selector (e.g., ‘Location’).
- Staff Label: The label shown above the staff selector (e.g., ‘Choose a Staff’).
- Service Placeholder: The placeholder text inside the service dropdown before a selection is made (e.g., ‘Select service’).
- Location Placeholder: The placeholder text inside the location dropdown (e.g., ‘Select location’).
- Staff Placeholder: The placeholder text inside the staff dropdown (e.g., ‘Select staff’).

2.2 Date & Time Step #
Customize the text shown on the step where customers pick their appointment date and time.
- Heading: The main title for the date selection step (e.g., ‘Date & Time Selection’).
- Subheading: A supporting description shown below the heading (e.g., ‘Select your appointment date and time’).
- Loading Text: The message shown while available dates are being fetched (e.g., ‘Loading available dates…’).
- No Available Text: The message shown when no dates are available for booking (e.g., ‘No available dates found’).

2.3 Time Slot Step #
Customize the text shown on the step where customers select an available appointment time.
Heading: The main title for the time selection step (e.g., ‘Available Time Slots’).
- Subheading: A supporting line shown below the heading (e.g., ‘Select your appointment date and time’).
- Loading Text: The message shown while time slots are being fetched (e.g., ‘Loading time slots…’).
- No Slots Text: The message shown when no time slots are available for the selected date (e.g., ‘No available time slots for the selected date.’).
- Bringing Anyone Label: The label for the guest/attendee field shown on this step (e.g., ‘Bringing Anyone?’).
- Bringing Anyone Sub-Label: A short description below the label to guide customers (e.g., ‘Add extra people to your booking’).
2.4 Details Step #

Customize the text on the step where customers enter their personal information.
- Heading: Main title for the details step (e.g., ‘Customer Details’).
- Subheading: Supporting description (e.g., ‘Customer Information’).
- Full Name Label: Label for the name field (e.g., ‘Full Name’).
- Full Name Placeholder: Placeholder inside the name field (e.g., ‘Enter name here…’).
- Phone Label: Label for the phone field (e.g., ‘Phone number’).
- Email Label: Label for the email field (e.g., ‘E-mail’).
- Email Placeholder: Placeholder inside the email field (e.g., ‘Enter e-mail…’).
- Note Label: Label for the optional note field (e.g., ‘Note’).
- Note Placeholder: Placeholder text inside the note field.
2.5 Payment Step #

Customize the text shown on the payment summary step.
- Heading: Main title for the payment step (e.g., ‘Payment’).
- Subheading: Supporting line below the heading (e.g., ‘Payment Summary’).
- Summary Title: The title of the appointment summary section (e.g., ‘Appointment summary’).
2.6 Checkout Step #

Customize the text shown where customers select their payment method and complete payment.
- Heading: Main title for the checkout step (e.g., ‘Checkout’).
- Subheading: Supporting description (e.g., ‘Make your payment’).
- Payment Method Title: Label shown above the payment method options (e.g., ‘Payment method’).
2.7 Confirmation Step #

Customize the text on the final screen shown after a booking is successfully completed.
- Heading: Main success message (e.g., ‘Appointment Confirmed’).
- Subheading: A warm follow-up message (e.g., ‘We Look Forward To Seeing You.’).
- Details Section Title: The title of the booking summary on the confirmation screen (e.g., ‘Appointment Details’).
2.8 Sidebar #

Customize the labels displayed in the left-side summary sidebar throughout the booking flow.
- Summary Title: The heading of the booking summary panel (e.g., ‘Summary:’).
- Customer Title: Label shown next to the customer name (e.g., ‘Customer:’).
- Help Title: Heading of the help section at the bottom of the sidebar (e.g., ‘Questions?’).
- Help Text: Support message below the help title (e.g., ‘Call %s for help’). Use %s as a placeholder — it will be replaced with your business phone number automatically.
All label and text changes reflect immediately in the Live Preview. Click Save Changes to apply, or Reset to restore all default text.
3. Button #
The Buttons tab lets you customize the text displayed on all navigation and action buttons throughout the booking form.

3.1 Button Labels #
Customize the text on each button customers interact with during the booking flow.
- Next Button: The label on the forward navigation button (e.g., ‘Next’).
- Back Button: The back navigation button (e.g., ‘Back’).
- Confirm Button: The label on the final payment/confirm action button (e.g., ‘Continue’).
- Processing Text: The message shown on the button while a payment or action is being processed (e.g., ‘Processing…’).
4. Feature Toggles #
The Feature Toggles tab lets you show or hide specific elements of the booking form without any code changes.

4.1 Visibility Toggles #
Use these toggles to control which form elements are visible to customers during the booking process.
- Show Note Field: Switch on to display the optional note/message field on the Details step, allowing customers to leave a message with their booking.
- Show Sidebar: Activate to display the left-side booking summary sidebar throughout the booking flow.
- Show Sidebar Illustration: Toggle on to display the decorative illustration image inside the sidebar. Requires Show Sidebar to also be enabled.
- Show Calendar Export Buttons: Switch on to display calendar export buttons (Google Calendar, Apple Calendar, Outlook, Re-Book) on the confirmation screen.
- Allow Back Navigation: Toggle on to allow customers to navigate back to previous steps during the booking process using the Back button.
