πŸ”€ Description of Change

agent_operations was landed in #14416 but nothing was writing to it β€” operation history still only lived in Redis with a 2-hour TTL. This PR wires the runtime so every operation gets persisted at its start and again at its terminal point.

Scope is intentionally minimal: only start-time and end-time writes. Per-step aggregate updates (afterStep) are deferred so we don't blow up Neon write volume on high-frequency operations.

What's wired

  • AgentOperationModel (packages/database/src/models/agentOperation.ts)

    • recordStart(params) β€” INSERT row with status='running', started_at=now(), ids/model/provider/trigger/appContext/maxSteps. Idempotent on the PK via onConflictDoNothing so resumed ops don't blow up.
    • recordCompletion(operationId, params) β€” UPDATE the row to its terminal state with aggregates, cost/usage jsonb, error, interruption, and trace_s3_key. Scoped by userId so a leaked operationId can't flip another user's row. No-op when the start row was never written.
    • findById(operationId) β€” userId-scoped lookup.
  • CompletionLifecycle owns both ends of the persistence lifecycle:

    • new recordStart() method called from AgentRuntimeService.createOperation before any state work.
    • new persistCompletion() invoked from inside dispatchHooks() before user hooks fire, so downstream consumers see the row in its terminal shape.
    • waiting_for_human is treated as a pause: status is updated but completed_at is left null so analytics doesn't read paused ops as completed. The next dispatchHooks call (when the human resumes and the op truly ends) overwrites both.
  • parentOperationId plumbed through ExecAgentParams β†’ OperationCreationParams β†’ recordStart so sub-agent invocations (execSubAgentTask) carry their parent lineage to agent_operations.parent_operation_id.

  • All DB writes are wrapped in try/catch and logged via debug β€” a DB hiccup never blocks the runtime startup path or the executor's terminal cleanup.

Not in scope (deferred follow-ups from the Linear issue)

  • Per-step aggregation via afterStep hooks (Neon write-volume concern).
  • Stale-row cleanup cron for orphan running rows.
  • TRPC query API for the UI.
  • agent_config sanitization β€” no agent_config column exists in the current schema; model_runtime_config jsonb only carries safe { model, provider }.

P2 Badge Defer start-row writes until startup cleanup can run

When createOperation is called with an already-aborted signal, or when createAgentOperation/scheduleMessage throws during startup, this new recordStart has already inserted a status='running' row, but the catch path only cleans up coordinator state and rethrows; dispatchHooks never runs and nothing updates or deletes the agent_operations row. Those failed starts are then persisted indefinitely as running operations even though execAgent reports them as failed/aborted, so place the insert after the initial abort check and ensure the startup failure cleanup marks or removes the row.
