Surfaced from agent-gateway error triage: an op_* failed with provider=lobehub / model=deepseek-v4-flash and the only logged signal was the bare V8 message:

Bad escaped character in JSON at position 160050 (line 1 column 160051)

No raw chunk, no stack, no provider/model on the propagated error payload.

🔀 Description of Change

When the OpenAI / Anthropic SDK iterator throws (most often a JSON.parse SyntaxError on a malformed SSE chunk — e.g. an upstream response containing an illegal backslash escape), convertIterableToStream in core/streams/protocol.ts previously only forwarded message/name/stack into the FIRST_CHUNK_ERROR payload. Downstream consumers (agent-gateway error log, in-app error renderer, Sentry, etc.) end up with a generic SyntaxError and no way to:

  • correlate the failure with which provider/model produced it
  • group identical upstream failures by parse offset
  • inspect the underlying error.cause that wrapped errors (e.g. APIError(cause: SyntaxError)) routinely carry

This PR threads optional { provider, model } context through convertIterableToStream / readableFromAsyncIterable and enriches the error payload with:

  • provider / model — from payload already present in every stream handler
  • parsePosition — extracted from V8 JSON SyntaxError messages (/position\s+(\d+)/)
  • causeName / causeMessage — when error.cause is an Error
  • cause — for non-Error causes, deep-cloned via structuredClone with String(cause) fallback

Threaded into OpenAIStream, OpenAIResponsesStream, and AnthropicStream. Other callers (qwen.ts, spark.ts, ollama, huggingface, zhipu) keep working with context = undefined — kept out of scope to keep the surface area small; can be wired up later if useful.

🧪 How to Test

Three new tests covering: provider/model enrichment, JSON parsePosition extraction, and error.cause surfacing. All 39 tests pass.

📝 Additional Information

Backwards compatibility: context is optional everywhere. Existing fields (message/name/stack) are unchanged. The existing test asserting chunks[1].message === 'rate limit' still passes unmodified.

What this does NOT fix: the raw upstream SSE chunk that failed JSON.parse is still discarded by the OpenAI SDK (it console.errors sse.data and rethrows the bare SyntaxError). Capturing the raw bytes would require teeing the fetch response body before handing it to the SDK — out of scope for this PR. The enrichment here gets us 80% of the diagnostic value (provider/model/position) without that restructuring.

Two review findings on #14636:

1. Wrapped SyntaxErrors lost their parsePosition. Provider SDKs commonly
   rethrow `JSON.parse` failures wrapped in their own error class
   (e.g. `APIError(cause: SyntaxError)`), so the outer `error.name` is
   no longer `'SyntaxError'` and the previous check skipped extraction
   for the exact case this enrichment was meant to diagnose. Now
   `extractParsePosition` walks both the outer error and any `Error`
   cause, and accepts any error whose message still carries the
   `"JSON at position N"` signature even if the SyntaxError name was
   lost in wrapping.

2. Cause cloning could blow up the entire diagnostic path.
   `structuredClone` succeeds on values that `JSON.stringify` later
   throws on (BigInt, circular refs), so a non-Error cause carrying
   either would surface as `payload.cause = clonedObject`, then the
   outer `JSON.stringify(payload)` would throw inside the catch handler,
   and the FIRST_CHUNK_ERROR chunk never gets emitted. Replaced with
   `safeJsonStringify` (BigInt → string, cycles → `[Circular]`) and
   route the cause object through `toJsonSafe` so the returned shape is
   always plain JSON.

Added tests for both: a wrapped APIError(cause: SyntaxError) yields
parsePosition, and a cause containing both BigInt and a circular ref
still emits a parseable error chunk.
