AIRQ® asthma refers to the Asthma Impairment and Risk Questionnaire, a 10-question, yes/no assessment tool used by healthcare providers and patients (12+) to gauge asthma control and predict future asthma attacks
By evaluating symptoms, medication use, and healthcare resource utilization over the past two weeks and past 12 months, the AIRQ generates a score indicating whether asthma is well-controlled, not well-controlled, or very poorly controlled.
The AIRQ has been validated in multiple languages (plain language summary can be found in George et al. 2025).

Purpose of the AIRQ
- Identify Uncontrolled Asthma: The AIRQ helps pinpoint patients whose asthma may not be adequately controlled, prompting further discussion with a healthcare provider.
- Discuss Asthma Control: It serves as a tool to initiate conversations between patients and their healthcare providers about current asthma control levels.
- Assess Exacerbation Risk: A higher score can indicate a greater risk of asthma attacks or exacerbations.
How the AIRQ Works
- 10 Yes/No Questions: The questionnaire asks 10 questions covering factors like symptoms, activity limitations, sleep disturbance, and the use of rescue medication.
- Impairment and Risk Items: Seven items focus on past two weeks of impairment (symptoms and activity), while three focus on the past 12 months of risk (medication use, symptoms, and exacerbations).
- Scoring: Responses are tallied into a score from 0 to 10.
- 0–1 Yes answers: Well-controlled asthma.
- 2–4 Yes answers: Not well-controlled asthma.
- 5–10 Yes answers: Very poorly controlled asthma
Important Considerations
- AIRQ is not a Diagnostic Tool: it does not diagnose asthma but assesses its control.
- Discuss with Your Provider: Patients should always discuss their AIRQ answers and scores with a healthcare professional to determine if treatment adjustments are needed.
- AIRQ questionnaire is designed for individuals aged 12 and above. The Peds-AIRQ is a recently validated assessment tool for children aged 5 to 11 years (Murphy et al. 2025)
