Abstract


  1. Diabetes is a long-term condition in which the body cannot maintain safe blood sugar levels due to insufficient insulin production or improper insulin use.

  2. Symptoms may go unnoticed at first but can include thirst, frequent urination, unexplained weight loss, tiredness, slow-healing wounds, and blurred vision.

  3. Family history, being overweight, little or no exercise, and eating unhealthy foods can raise the risk of diabetes.

  4. Regularly checking your blood glucose helps you understand how food, activity, and medications affect your body and allows your healthcare team to adjust your treatment plan.

  5. Diabetes is treated with pills or insulin when needed, but a healthy lifestyle always comes first.

The Basics of Diabetes

According to the National Diabetes Statistics Report (2021), about 11.6% of the United States population, i.e., 38.4 million people, have diabetes. That makes diabetes one of the most common household diseases today. [1] Diabetes mellitus is a chronic disease in which the body has too much sugar (glucose). This happens either because the pancreas (your “insulin factory”) does not make enough insulin or the body cannot use insulin properly.  Insulin is a hormone that acts like a key, helping glucose move from the bloodstream into the body’s cells for energy. [2] There are two types of diabetes in general. Type 1 Diabetes, where your body does not produce insulin, and Type 2 diabetes, where your body doesn’t use insulin properly. Gestational diabetes is detected during pregnancy during a routine blood sugar or oral glucose test. [3]

The Hidden Triggers of Diabetes

Diabetes usually develops from a combination of factors rather than a single cause. In the community, we see that genetics, lifestyle, and environment all play a role. Stress, poor sleep, and even exposure to certain chemicals like pesticides or hormone-disrupting substances can also raise risk. Together, these factors help explain why diabetes has become such a widespread health problem in our communities.

1. Genetic Factors

Genetics can influence the risk of type 2 diabetes by affecting how the body produces and uses insulin, the hormone that controls blood sugar. [4]

2. Ethnicity

Certain ethnic groups, including African Americans, Hispanics, Native Americans, and   Asian Americans, have a higher risk of developing diabetes.

3. Lifestyle Factors

Several lifestyle factors increase the risk of developing type 2 diabetes.

  • Obesity: Extra body weight, especially around the belly, makes it harder for the body to use insulin.  Obesity can cause insulin resistance, a condition in which the body’s cells do not respond properly to insulin.
  • Physical Inactivity: Not getting enough physical activity can make it harder to maintain a healthy weight and can affect how the body uses insulin. Regular exercise can help improve insulin sensitivity and support overall health.
  • Diet: A diet high in processed foods, sugary beverages, and unhealthy fats increases the risk of Type 2 Diabetes. Conversely, a diet rich in whole grains, fruits, vegetables, and lean proteins can help reduce this risk.
  • Age and Family History: The risk of type 2 diabetes grows with age especially after 45. It is more likely to have a genetic connection if there is a family history of diabetes. [5]

Does Diabetes Always Cause Symptoms?

No, diabetes does not always come with obvious symptoms. Many people with type 2 diabetes go for years without noticing anything, even as blood sugar levels rise. This is especially true in the early or latent stages of the disease. [6] When symptoms do appear, they are caused by high levels of glucose in the blood and may include :

  • Excessive thirst.
  • Excessive hunger.
  • Frequent urination.
  • Fatigue.
  • Frequent infections.
  • Unintentional weight loss.
  • Signs of poor circulation, including tingling or numbness in the hands and feet (diabetic neuropathy).
  • Wounds that do not heal.
  • Blurred vision.

Many people with diabetes do not notice any symptoms so it is very important to check regularly. Blood tests such as fasting glucose or A1C can find diabetes early and show how well it is being managed. Keeping track of blood sugar helps doctors create a plan that fits each person’s needs including changes in diet, exercise, and medications. This approach keeps blood sugar steady prevents sudden highs and lows and lowers the chance of long-term health problems.

Numbers That Matter: How Sweet is Too Sweet?

Even when there are no obvious symptoms, simple tests can show how the body is handling blood sugar. These tests help doctors understand risk, detect diabetes early, and guide the best plan for managing it.

  • Random Blood Sugar Test: It is a test that measures blood glucose at any time, regardless of when the person last ate. If the blood sugar level  is 200 milligrams per deciliter (mg/dL) or more, you may have diabetes
  • Fasting Blood Sugar Test: After overnight fasting, a blood sample will be taken. A fasting blood sugar level of less than 100 mg/dL (5.6 mmol/L) is normal. A fasting blood sugar level from 100 to 125 mg/dL is considered prediabetes. If it is  126 mg/dL or higher on two separate tests, you have diabetes.
  • Hemoglobin A1c (HbA1c): This blood test shows your average blood sugar levels over the past two to three months. An HbA1C between 5.7% and 6.4% means that you have prediabetes, while below 5.7% is considered normal. If you have an HbA1c of 6.5% or higher on two separate tests, you have diabetes. [7]

Treatment of Diabetes is Not the Same for Everyone

Diabetes care is highly personalized, so no two treatment plans are exactly the same. Not everyone with diabetes needs insulin. Some people can manage blood sugar with oral medications alone, while others may benefit from daily insulin injections, once-weekly long-acting insulin, or newer options like long-acting Glucagon-like peptide-1 (GLP-1) receptor agonists.

The right treatment depends on blood sugar levels, other health conditions, and a doctor’s guidance. Talking with the healthcare team about what works best is important. No matter which medications are used, a healthy diet is always the foundation of diabetes care. A nutritionist can help improve eating habits and make any treatment plan stronger and more effective.

5 Key Facts About Diabetes Treatment

  1. Not everyone needs insulin. Some people manage diabetes with oral medications and lifestyle changes alone.
  2. Insulin is always required to treat type 1 diabetes. It is important to note that oral medications do not work for type 1 diabetes.
  3. In type 2 diabetes, oral medications are prescribed if diet and exercise do not improve blood glucose control after a few months. Metformin is usually the first medication prescribed for type 2 Diabetes. It helps lower blood sugar levels and has been shown to reduce the risk of complications. If metformin alone isn’t enough, your doctor may add other medications such as sulfonylureas, dipeptidyl peptidase-4(DPP-4) inhibitors, GLP-1 Receptor Agonists, or sodium glucose cotransporter two (SGLT-2) inhibitors. These medications work in different ways to help regulate blood sugar levels. Your doctor prioritizes SGLT-2 inhibitors for patients with heart failure or chronic kidney disease and GLP-1 Receptor Agonists for those at high risk of cardiovascular events or with a goal of weight loss. If you have stage 4 or more chronic kidney disease, your doctor may stop prescribing your metformin.
  4. You may also require insulin to treat Type 2 Diabetes if diet, exercise, and oral medications do not improve blood glucose control. Tell your doctor if your HbA1c levels are unmet and check to see if you need insulin. [8,9]
  5. Continuous Glucose Monitors (CGMs) track blood glucose levels in real-time, helping patients and doctors fine-tune their treatment. You can see your blood glucose control without any daily pricks from your smartphone. If you have Type 1 diabetes or uncontrolled Type 2 diabetes, you may benefit from CGMs that monitor your blood glucose levels regularly. [9]

5 Tips for Monitoring Blood Glucose at Home

  1. You will need a glucometer to measure your blood glucose levels at home. How often you check your blood glucose level depends on your diabetes type, control, and whether you are taking insulin or not.
  2. Blood sugar levels are expected to range between 90- 120 mg/dl when fasting and less than 140mg/dl after a meal (postprandial) when your diabetes is under control. We recommend checking your blood sugar at least twice daily and maintaining a logbook of your blood sugar levels.
  3. Consistently logging your blood sugar levels in a diary can help inform your medication regimen for your primary care provider or endocrinologist.
  4. Blood glucose levels can be checked before and after meals, before and after exercises, early in the morning, and before bed.
  5. If you have a new insulin routine, a change in medications, or a new work schedule, check your blood glucose more frequently. Talk to your doctor if you see low blood glucose levels. [9]

  • Wash your hands with soap and water.
  • Please do not wash your hands with alcohol, as it dries the skin.
  • Get the lancet ready. Always use a fresh lancet.
  • Always place a new test strip into the meter. Ensure the strip and meter codes match.
  • Prick your fingertip with the lancet in the protective lancing device. Do not prick the same finger every time you measure your blood sugar.
  • Squeeze from the finger's base. Carefully place a drop of blood onto the test strip.
  • The test results will appear on the screen of the glucometer. [9]

HbA1C: The 3-Month Report Card for Your Blood Glucose

HbA1c is a blood test that shows average blood sugar levels over the past two to three months. HbA1c can help diagnose diabetes or monitor how well your diabetes treatment plan is working. Your primary care provider can use it to formulate a game plan to manage your diabetes. It is best to get HbA1c tested at least twice every year. If you're not meeting your goals or changing treatments, you may need an HbA1C test more often. The American Diabetes Association recommends a target A1C level of 7% for non-pregnant adults. [7]

HbA1C in Older Adults: One Size Doesn’t Fit All

Elderly patients with various medical conditions can have a higher target of less than 8%. [7,10,11] The health benefits of lowering HbA1c levels to below 7 % in older people are uncertain. Please note that lowering HbA1c may increase your risk for hypoglycemia. When you age, your kidneys become less efficient, which causes insulin (or other drugs) to accumulate in your body. Older people also often take multiple medications, some of which may interact with diabetes medication and contribute to hypoglycemia.  Hypoglycemia (severely low blood sugar) can result in confusion, coma, falls, hospitalizations, and even death. [12] Because every person is different, there is no single HbA1C target that works for everyone. Your age, overall health, and other medical conditions all affect what goal is right for you. That’s why it’s important to discuss your personal A1C range with your doctor. Together, you can decide on the safest and most effective plan that works for you.

Small Changes, Big Impact: Lifestyle Steps for Diabetes

If you’re living with type 2 diabetes, there’s a lot you can do to stay healthy and avoid complications. Simple lifestyle habits keep blood sugar steady and protect the heart, kidneys, eyes and nerves giving the body the best chance to thrive.Life with type 2 diabetes can still be full and healthy.

  • Maintain a Healthy Weight: For people who are overweight, even a small weight loss of 5 to 7% of body weight can improve blood sugar levels. This change also reduces insulin resistance and helps medications work more effectively.
  • Be Physically Active: Try to get about 30 minutes of movement most days of the week, like walking, cycling, or swimming. Physical activity helps lower blood sugar and boosts energy, mood, and heart health.
  • Eat Balanced Meals: Focus on foods good for your blood sugar, like vegetables, fruits, whole grains, lean proteins, and healthy fats. Try to limit sugary drinks, processed snacks, and foods high in salt. Examples of healthy diets include the Mediterranean diet and the Dietary Approaches to Stop Hypertension (DASH) diet.
  • Quit Smoking if You Smoke: Smoking makes it harder to manage blood sugar and increases your risk of heart disease, stroke, and other complications. Quitting is one of the best things you can do for your health.
  • Take Your Medications and Check Your Blood Sugar: Follow your doctor’s instructions, take medications as prescribed, and monitor your blood sugar as recommended. This helps you catch changes early and adjust treatment if needed. [13]

What Happens if Diabetes is Not Controlled?

If you always have High blood glucose levels, you are at risk for serious health problems such as heart disease, stroke, kidney failure, vision loss, nerve damage, skin ulcers, and infections. Regular eye exams are essential to detect vision changes early, when treatment works best. It’s also important to check your feet daily for cuts, blisters, or sores because with nerve damage, you may not feel them until they become severe. The best way to protect yourself from these complications is to lower your blood sugar and keep your HbA1C within the target range set by your doctor. [14,15]

At What Point is Low Blood Sugar Critical?

Hypoglycemia means your blood sugar has dropped too low. This is a life-threatening emergency. You will have symptoms of hypoglycemia when your blood glucose is less than 70 mg/dL. If you are at home, you need to act now.

You can check your blood glucose levels to ensure you are not experiencing low blood glucose. If you are experiencing symptoms of low blood sugar and cannot check your blood glucose for any reason, treat hypoglycemia instead. You can do this by taking glucose tablets, drinking juice, or eating food that can raise your blood sugar level. Your doctor might prescribe glucagon, which can be given as an injection by a family member if you are unresponsive at home. If you have frequent hypoglycemia attacks, seek medical attention. If you take a medication that can cause hypoglycemia, your doctor can adjust your medication dose. [17]

  • Feeling shaky and irritable.
  • Sweating, chills, and clamminess.
  • Confusion.
  • Feeling lightheaded or sleepy
  • Feeling weak or having no energy.
  • Blurred/impaired vision.
  • Headache.
  • Coordination problems, clumsiness.
  • Seizures or loss of consciousness.
  • Rapid pounding of the heart or irregular heartbeat. [16,17]

Who Needs an Insulin Pump? Here’s What to Know

Insulin pumps are additional devices attached to your body that are programmed to mimic your body's normal release of insulin. The pump can deliver insulin continuously or as a surge (bolus) close to mealtimes.  This delivery method can control the rise in blood sugar after a meal. Using a pump doesn't mean you no longer have to check your blood sugar. It can take some time to get used to the pump. Discuss with your healthcare provider whether an insulin pump is the best option for you. Check with your insurance provider if your doctor determines that an insulin pump is a good option for you. Most insurance providers cover insulin pumps, but sometimes they may not be covered and can be expensive. [18]

Conclusion

Diabetes is a long-term condition, but it can be controlled with the proper care. Getting diagnosed early, checking your blood glucose regularly, and taking your medicines as directed all help prevent serious complications. Healthy habits, such as eating nutritious foods, staying active, maintaining a healthy weight, and avoiding smoking, are equally important. Diabetes care isn’t only about blood glucose: it is about caring for your whole body and living well.

  1. How often should I see an eye doctor or a foot specialist?
  2. Do I need to decrease my insulin dose when feeling ill or when I am fasting?
  3. What if I forget to take my diabetes pills and remember later? Can I still have them?
  4. Will the diabetes medications interact with other medicines I am taking?
  5. Are there any specific considerations for managing diabetes as I age?
