- Our #JACC letter shows troponin release is common following exercise in patients with a severe coronary stenosis. ⬆️exercise intensity, ⬆️troponin increment PCI markedly⬇️ischaemia, but no effect on troponin release compared with placebo. Suggests ischaemia not the mechanism

- #ISCHEMIA and #ORBITA are not trials of Revasc vs. OMT (common misconception). Both arms get "OMT", so the trials examine the *additive* benefit of Revasc on a background of medical therapy. Here's why assessing quality of medical therapy is critical: doi.org/10.1161/CIRCOU…

- This is the principal question of the ORBITA-STAR trial, coming in the next year @rallamee @ImperialNHLIPCI is effective at reducing angina when the target coronary lesion is responsible for angina. Sounds obvious, but bears repeating because of the implications: This is a DIAGNOSTIC problem, not a therapeutic one. We don't have a perfect test linking symptoms to disease.

- Having spent an unhealthy amount of time together making #ORBITA2 happen, I can confirm that these are the TWO smartest people on the planet @mshunshin @rallameeThere is an unsung hero in the ORBITA family and we don’t do forgotten heroes. @mshunshin is the cleverest man I know and responsible for all the statistics and beautiful figures in ORBITA-2 (with a little help from my RAL chart). Give this man a round of applause!
