    Seeing a patient smile when they realise they can swallow is amazing. Pt with Ca+ tracheoesophageal fistula.Tried to cross from the mouth but wire would only exit through fistula into the trachea. Pt desperate to swallow. Came up from a gastrostomy I'd prev sited. @DrAidanShaw

    Fridays are biliary day! Occluded plastic stents are a challenge. I find the neatest way to remove them is to puncture the duct above them, cannulate the middle of the stent, & use a balloon inflated inside to push them into the duodenum @DrAidanShaw
