VA Nexus Evaluation Intake Form
For Veterans Seeking Nexus Letters or DBQsThis form is for veterans who are ready to begin the nexus letter or DBQ evaluation process. It collects your military service history, claimed conditions, service connection explanation, and allows you to upload supporting records directly.
Use this form if you are:
- Filing or appealing a VA disability claim
- Seeking a nexus letter for service connection
- Requesting a DBQ, remote or in-person
- Ready to submit records for Dr. Baca's review
No payment is collected at submission. Dr. Baca will review your intake and contact you within 3–5 business days with a determination and fee quote if a nexus opinion can be supported.

Once your intake form is received, Dr. Baca reviews your health history before your appointment. Your first visit includes a thorough physical and orthopedic evaluation, range-of-motion assessment, and discussion of your symptoms, history, and goals.
Imaging is ordered when clinically indicated. Your individualized treatment plan is explained at the end of your initial visit so you know exactly what to expect from your care.
