What Is a VA Nexus Letter?


A VA nexus letter is a medical opinion that explains how a veteran's current diagnosed condition is connected to military service. For many VA disability claims, the Department of Veterans Affairs requires evidence showing that a condition is "at least as likely as not" related to service.

That connection is the nexus. A well-supported nexus letter explains how your military duties, injury history, service records, and current diagnosis are medically related — using VA-standard language and clinical rationale.

Nexus Letter vs. DBQ

A nexus letter establishes the service connection between your condition and military service. A Disability Benefits Questionnaire (DBQ) documents the severity, symptoms, and functional impact of a condition. Both may be needed depending on the stage of your claim.

How the Process Works


The evaluation process is straightforward. No payment is required until Dr. Baca has reviewed your case and confirmed a nexus opinion can be supported.

Step 1 — Submit Your Intake

Complete the nexus intake form directly, or submit a brief inquiry first if you want to confirm your case is a good fit before completing the full intake.

Step 2 — Records Review

Dr. Baca reviews your service records, VA records, private medical records, imaging reports, denial letters, and other supporting documentation you provide.

Step 3 — Determination

You are contacted within 3–5 business days with a determination. If a nexus opinion can be supported, you receive an invoice and payment link to proceed.

Step 4 — Payment

All services must be paid in full prior to delivery of any nexus letter or DBQ. Payment is not collected at the time of intake submission.

Step 5 — Letter Preparation

Dr. Baca prepares a detailed nexus letter with clinical rationale using VA-standard language. A DBQ is added if requested and appropriate.

Step 6 — Delivery

Your completed nexus letter and/or DBQ is delivered to you electronically. Most letters are completed within 7–10 business days after all records are received.

Each case is evaluated individually — a favorable opinion can only be provided when medically supported by the records and clinical findings.
What records do I need to provide?
The more documentation you can provide, the stronger the evaluation. Helpful records include your DD-214, service treatment records, VA medical records, VA decision or denial letters, private medical records, and imaging reports (MRI, X-ray, CT). Buddy statements and prior DBQs are also useful. You upload your records directly through the intake form.
