My understanding of what was wrong here:
WorkspaceDb::insert_source()is supposed to look for an existingDocumentFileSourceand return its index.file_sourcesand return the new index.boxcar::Vec, the iterator yields(index, &value), whereindexis the actual stable source index.position()ignores this actualindexand instead returns the current count of slots.boxcar::Vec, when an insertion is in progress or when the internal storage contains inactive slots. see 🐛 The file /home/repo/biome.json does not exist in the workspace #10268 (comment)What this fix does:

Test Plan
I struggled with this. Reproducing the error itself was especially challenging with it being a race and I never managed to do it locally, only in CI on a machine with 60+ cores. If there's a suggestion for how to write a test on this, I'd be happy to author one.
