`LegacyModuleIdx` (enum, its inherent impl, and both `From` impls) has no callers anywhere in the workspace. Removing it as dead code, along with its module declaration and re-export. The real `ModuleIdx` lives in `ecmascript/module_idx.rs` and is unaffected.
