If clinical academics were a species we would be on an endangered list, and very high up it.

Junior doctors Vs Minimum Wage The break even point unlikely to be 13y (age 31) post if any loans needed for living expenses during medical school, a problem compounded if any of those loans accumulate interest You'd be a consultant in the US by then Btw theguardian.com/society/2023/a…

If you don't work in the NHS doctors training, pay & career progression is a bit confusing Now the #JuniorDoctorsStrike is on you might reasonably ask what does time served really lead to? So in this thread I'm going to compare doctors to our dearly beloved 650 MP's 1/

15 years ago I had to call an emergency plumber out on the Easter Bank holiday to fix a burst pipe. He charged £100/h That's about twice the NHS wage bill for fixing a burst blood vessel in a living person.... Today

1/ We’ve done a thing I think it's the most important paper I've contributed to date, or may ever do This🧵is a bit longer for the non-experts, it gets good at the end It is about how we measure calcium (Ca2+) in ❤️ cells Open access ⬇️ dx.doi.org/10.1016/j.yjmc…

From the cardiac surgical team @osaka_univ where I briefly worked with @Lab_Nagai @NadimMahmud2 @Shigenori774 @HajimeS10 in 2017 Seriously can't understand why this didn't make @NEJM, I guess they will only publish the case series academic.oup.com/ejcts/article-…

I have been told many times (from both sides of the debate) that someone with a basic science focus shouldn't have any aspirations to do cardiac interventions, or lead such services Well, I am sorry to have ignored these words of wisdom I'll let the pictures speak for me P.F.O

My 16 year personal ambition to repair the heart with cells got a big step closer today. I'm starting as SANA Executive Director for Cardiac Cell Therapy, Seattle, WA. Grateful for help from @euanashley @cesterr @JimJohnsonSci @Lab_Nagai (+ others not on twitter) to get here!

This is a first year doctors payslip ⬇️ 1) they aren't paid much 2) their tax code is wrong after they had to rotate between hospitals (plus cover whatever costs that involves) so a huge tax deduction is made This is the lived experience in the modern NHS It shouldn't happen
This is a payslip of a FY1 doctor in the NHS. They are paid £14 an hour. They take home £1097 a month. 70k+ in debt for student loan after 5/6 years of medical school. This is not fair. You can't blame doctors for leaving. #PayRestoration @BMA_JuniorDocs @doctors_vote

This is unreal Having failed to get 93% of cancer patients seen by the 2 wk window since 2018 (i.e. it was working up to 2018) we will now have "targets" that ask for a 75% hit rate by 28 days & treatment by 2 months It's not good, it's not acceptable & it's not 1984
The Govt will announce plans to scrap the 2 week urgent cancer wait time and nine other cancer targets on Thurs in favour of 3 new targets the NHS believes will lead to faster diagnosis and treatment: thetimes.co.uk/article/8b06c8…

A few other differences: Probity: optional Gross negligence: not dismissible Duty of candour: optional Recruitment training: personal knowledge from school or via financial services support preferred Public trust: nonexistant Ethics: can't be trusted, need formal advisor

Students: why do they still teach glycolysis & Krebs cycle? Doctors: I never use Krebs cycle or glycolysis, dunno why they teach it Patients: how does FDG-PET find infection, cancer or ❤️? Students: 🤷 Doctors: 🤷‍♀️ but in a white coat Metabolic medicine: hold my beer...
I’m in the minority here but I think the Krebs cycle is not taught enough, or it’s not taught in a way that truly explains why it is the centre of life on earth. I totally disagree it is irrelevant to being a doctor. But I do agree testing rote knowledge of each step is pointless

I was asked to talk about post myocardial infarction VSD for #SCAI2023 recently. In 8 mins I was a bit limited, so here's an expanded thread Apologies that the movie compression makes things a bit jumpy, & if Twitter messes up the movie cropping I will add again at the end 1/
