    Pt with MVP and frequent PVCs from AL pap leading to decline in EF. Catheter RFA with PVCs reduced 30%-10% and improved EF. Progressed to severe MR. Cryoablation of AL pap during MVr by expert surgeon @tomcnguyen. Prior RF lesions visible. Thankful for collaboration @HeartUCSF

    Heading home from #HRS2022. My highlights: -gathering with former ⁦@UCSF_EP fellows; PFA promising but not perfect… so many companies working on this- who will survive? PFA promising for VT; LBB pacing; #bringbackposters; so many enthusiastic fellows- need more women in EP!

    Late night mapping redo-redo "AVNRT" with Sonny Jackman, Mel Scheinman, Henry Hsia and UC team. Turned out to be slowly conducting concealed AP - could only map using @True_EP "Sauer" maneuver. Successful RF from L fibrous trigone. Always plenty to learn in EP!

    29 yo with syncope, then resuscitated from VF arrest. EPS shows VF reproducibly initiated only by PACs (not with V-stim). Always initiated with aberrancy. SQ ICD shows shock triggered by single PAC. Going to lab today for RFA. Anyone seen anything like this before?

    Why may PW or LAA electrical isolation improve outcome in pers AF? Simultaneous endo-epi mapping in a swine model shows that endo-epi dissociation is greatest in these regions - 1 possible explanation. @fitzroytomos @DrGeoffLee @BoatNoodleSoup DOI: 10.1016/j.jacep.2020.04.015

    #EPEEPS Asked by device clinic whether to start NOAC for new AF. Looks like RNRVAS triggered AF. Hopefully fixed with reprogramming pacer. Under-recognized cause of AF?
