As an Independent Living Specialist with a focus on Nursing Home Transition at RTI, you’ll be at the heart of empowering people with disabilities to reclaim their independence and shape their own futures. In this role, you’ll work directly with individuals who are ready to transition out of nursing homes, as well as those who want to remain in their homes and communities. Your support assists people who live where they choose, with dignity, autonomy, and confidence.

As the NCW Program Coordinator at RTI, you'll play a key role in supervising 3 to 5 case managers that transitioning individuals with disabilities out of nursing homes and supporting them in setting and achieving their independent living goals and maintaining their independence in the community.

We have several programs that deal with Assistive Technology (AT) like wheelchairs, ramps, walkers, stairlifts, low vision aids, vehicle lifts, etc. We have a Loan Bank of AT and medical supplies, most of the AT has been donated and we clean it up and get it back out to people free of charge. Some of this AT we giving away to those who need it long term and for others we find alternative funding sources like grants, foundations, insurance to cover the cost of the items. There are NO COSTS for this service but donations are appreciated and are tax deductible.
Yes, we are a nonprofit 501(c)(3) tax exempt organization. Donations and in-kind support is tax deductible. Feel free to look us up through the Utah Department of Commerce at https://dcp.utah.gov/ or at IRS at https://www.irs.gov/
Yes, you can donate funds through (PayPal link)_____ or you can donate Assistive Technology equipment, medical supplies, recreation supplies, etc. at our office during office hours.
We love having the community involved in our programs and have several ongoing volunteer opportunities available. We can always use people to help in our reception area to take and make calls, help consumers and enter data in our computer system. We also have opportunities for assistance in our loan bank program to repair and clean equipment so they can be loaned out to those in need. Our recreation program welcomes volunteer to help with activities including our annual campout, fourth of July BBQ, holiday dinner, and other events. Currently we are looking for volunteers to assist us with our Capital Building Campaign to assist with outreach activities and raising funds.
We recommend calling ahead to either make sure we have the equipment or we have a staff member that can meet with you and talk about services, but anyone is welcome to come in without an appointment and talk to staff about services and supports.
Yes, we primarily provide rides to those coming to our events, activities, outings, and appointments. We also have transportation vouchers (link to transportation tab). However, we encourage whenever possible to ride and use public transit. We also will work on a case by case basis to provide transportation services when we can’t find transportation services or when people are waiting to become eligible for services.
