Expired U.S. passports are no longer valid for return travel after June 30, 2022. Please read below for information on how to renew your current passport or to request a temporary emergency passport.

Processing times vary depending on demand. If you are traveling within the next two weeks, please see the Emergency Passports section. Please note passport services are not available on weekends or on Peruvian or U.S. holidays.
You should complete this form if you’re applying for the first time, or you don’t meet our requirements to renew your passport.
You should complete this form if you’re applying for your child who is under age 16.
You should complete this form if one parent or guardian cannot go with the child to apply in person for the child’s passport. The parent or guardian should submit this special form in addition to their child’s application (DS-11).
You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11. As of February 27, 2022, the U.S. Embassy in Lima is able to receive certain adult passport renewal applications by mail. Currently, the mail-in renewal option is only available at certain DHL drop-off locations in Lima and Arequipa.
You should complete this form if you lost your passport or it has been stolen. To replace a lost or stolen passport, you must apply in person and submit Form DS-11 in addition to Form DS-64.

You should complete this form if you’re applying for your child who is under age 16.
Applicants Age 15 or Under Without Both Parents Present
You should complete this form if one parent or guardian cannot go with the child to apply in person for the child’s passport. The parent or guardian should submit this special form in addition to their child’s application (DS-11).
Adult Passport Renewal (DS-82)
You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11. Report and Replace Lost/Stolen Passport (DS-64 & DS-11)

Payments must be in the exact amount, and you must not use any other exchange rate other than what is listed below. Our internal exchange rate changes several times per year, with little notice.
You must provide one photo on a white background with your passport application. Our photo policies apply to adults and children. We have a few exceptions to our photo policies for infants.
If you are a U.S. citizen with an urgent need to travel to the United States, please write to LimaACS@state.gov and include your current passport information, an explanation, evidence of your reason for travel, and a contact phone number. Emergency appointments will be considered on a case-by-case basis. Having purchased airline tickets without a valid passport does not constitute an emergency. Do not purchase a flight ticket until you have a valid passport.
