The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Renew an existing passport issued to you when you were aged 16 or older.

Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Report a passport lost or stolen and replace it.
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo with your passport application. All our photo policies apply to both adults and children under age 16. We have a few exceptions to our photo policies for infants.
If your passport is lost or stolen, you are advised to report the loss/theft to the local Croatian police in the area where it occurred. In order to replace the passport, you must appear personally at the Embassy. You should submit your application for a replacement passport as soon as possible after the loss or theft is discovered.

We return new U.S. passports by mail ONLY (within the territory of Croatia). You must bring an addressed padded envelope with sufficient postage (Croatian Post registered mail, 3 Euro in postal stamps) to your passport appointment, and/or include it with your mail-in application.
IMPORTANT: Please, DO NOT send money for postage.
