About Us

About CHAN

Community Health Action Network (CHAN) is a nonprofit organization based in Victorville, California, serving families throughout the High Desert region. Since 2007, CHAN has been committed to one core mission: helping people help themselves.

We provide hands-on support, practical education, and community-based services that empower individuals to move from crisis to stability and beyond. Our programs are built for real life focused on what families truly need and grounded in the belief that lasting change starts with access, trust, and community participation.

A Different Kind of Help

CHAN doesn’t offer one-time handouts. We build long-term pathways.

Our model is based on participation. That means when people show up consistently and with intention we show up even stronger in return. The more a participant engages with CHAN programs and events, the more opportunities they receive for continued support, leadership, and growth.

Community Advocacy

CHAN works with local agencies, schools, nonprofits, healthcare providers, and government organizations to advocate for the services that matter most to our community. These partnerships help expand access to resources and amplify the voices of those often left unheard.

Community Engagement

We believe people should be active participants in the support they receive. Our programs are structured to encourage consistent engagement—building stronger, more resilient communities from the inside out.

Outreach And Education

Every class and service we offer is designed to help families gain practical knowledge and long-term stability. Current programs include:

  • Emergency Assistance Program (EAP): Utility help, food vouchers, and car seat safety education
  • Ubuntu Roots: A parenting program
  • Nutrition Classes
  • Financial Literacy Workshops

Civic Engagement

CHAN supports residents in learning how to advocate for themselves and their communities. Through events, partnerships, and community conversations, we help build bridges between people and policymakers.

Workforce Development

By encouraging volunteerism, skill-building, and consistent involvement, CHAN helps participants build confidence and job-readiness. We create space for community members to grow into leadership and employment opportunities through their own dedication.

Black Equity Initiative

CHAN is committed to addressing racial disparities in access to health, financial, and family support services. Through education, outreach, and culturally responsive programs, we work to uplift Black families and challenge the systems that have historically held them back.

Participation Model

Our services are built on mutual effort and respect.

To access programs, all clients must complete an intake form. Services are offered based on resource availability, and those who remain actively engaged in CHAN’s programs receive priority for future offerings.

Participants who volunteer, attend classes, and stay involved are given preference for:

Emergency assistance

Food and gift card distributions

Holiday giveaways

Invitations to special programs

Employment referrals or training opportunities

Participation Model

Our service model is designed to reward our clients for remaining involved with the organization.
CHAN is a non-profit supported by grant funding and donations. All applicants must complete an intake form at the time of request for services. Initial requests for emergency assistance will be accommodated based on resource availability.
Upon subsequent requests for assistance, clients will be provided with opportunities for volunteering or attending CHAN-sponsored events. Participants who volunteer or attend such activities will be given preference when requesting emergency items like food assistance, baby supplies, or gift cards.
The more clients invest in the program, the more they will be rewarded. Community members who demonstrate dedication to CHAN programs may also be considered for employment opportunities with our organization or our partners.

Participation Model

Our service model is designed to reward our clients for remaining involved in the programs. Although anyone can apply for our services, we grant priority to families with children under the age of 5.

Upon subsequent requests for assistance, clients will be given information for volunteering or attending CHAN-sponsored events. Residents who volunteer or attend such activities will be given preference when requesting emergency items like vouchers, food, baby supplies, or gift cards.
The more clients put into the program, the more they can receive. Community members who demonstrate dedication to CHAN programs and services may even be considered for paid positions with our organization or our partners.
