- I have moved to an endo first approach for ALI. I’m often asked when would I still do an open embolectomy. Here is a case in which I went open. Popliteal and three vessel occlusion with motor loss. (R2B) knowing fasciotomies will be required. Embolectomy of pop and all 3 tibials
