A friend's wife, GI, did a colonoscopy on a patient. The patient has electronic access to the procedure report, calls her, is irate, saying how she is an adult, and was well behaved during the procedure and did not need a "time out".

- What do you when your cardiorenal pt is hypoNa, oliguric despite 3 diuretics (loop, thiazide and MRA?) You "pandiurese" the nephron by blocking two more segments (acetazolamide & tolvaptan, now "quintuple-uretic"). And the pt diureses 2.5 L, Cr 3.0->2, Na 120-130! @juanstathop2
- 55 y/o found unresponsive at train stop. 911 to ED, he wakes up a bit, slumps over and says he lost vision in one of his eyes. Follow the case with me. Some of this is very simple, some not so (all values in mg/dl, mmol/l etc). @RUSHIMChiefs @RushTweetorials @RushDOIM.

- Today Burton ("Bud") Rose would be 80! Dr. Rose's books and teaching inspired so many of us. He was the gold standard. We miss him but are so thankful he was on this earth to contribute as much to nephrology (and med. education: UpToDate was his creation) as anyone. @BookBurton
- FSGS, MPGN, and TMA (among others) are finally recognized as a pattern of injury as opposed to a specific disease. For TMA this figure says it all
- You encounter a woman with HTN and hypokalemic metabolic alkalosis. First move? Renin and Aldo, Genetic testing, U chloride, Renal artery imaging, better history? The answer is always history. Huge kudos to prior @Rush_Nephrology fellow @MeloneyOliveira for making the diagnosis!
- Replying to @IBressendorffNice! Young man presented with ESRD, on HD his toes and fingers did this. I was clueless but my fellow came back from Kidney week (1989) learning of primary hyperoxaluria that becomes "malignant" oxalosis once you become anuric. Took a liver biopsy sent to UK to make diagnosis.
- 80 cases of hydralazine induced vasculitis! I remember the first one I saw 25 yr ago. Wasn't even on our radar. Now seems epidemic. How did this lousy - side effect laden - tid drug rise from the grave? Cardiologists?
- The @RushMedical newly opened #COVID19 8K ICU A-Team. Day 1 was amazing experience. Great support all around! My learning curve has higher slope than Covid pt # curve which means we will win. Picture at 7 PM handoff. Not shown are amazing residents from surgery, IR, anesthesia!
- After 24 yrs as @Rush_Nephrology Training Program Director I have decided to hand over the torch to @Proximal_Baxi. It took me forever to make this decision but now having done so I realize it was 100% the right person and 100% the right time! Congratulations Pravir.
