    New solutions -> new complications. #CardiacSurgery indicated: TAVR in mitral ring 26mm S3 well seated, treated MR and severe heart failure … but MS developed due to thrombus on one cusp (shown). Replaced with 31mm bioprosthesis, recovered well @ClevelandClinic HVTI #TAVRTeam

    Interesting thread discussing root/anulus enlargement in AS vs small LVOT - some need both anulus & LVOT addressed & for that we @CleClinicHVTI have preferred an anterior modified Bentall w/ Single Patch Konno Enlargement (BSPoKE) technique as shown #AortaEd
    You are absolutely correct. All these iterations do not represent an annular enlargement. They simply allow you to tilt the valve up toward the neo-aorta, thus allowing you to fit a larger valve. The LVOT is still the area-limiting factor, whether you squeeze in a 23 or a 27
