Can I have a caregiver in Illinois?
In Illinois, a registered designated caregiver can assist the registered qualifying patient with the use and purchase of medicine. A registry identification card is required to enter a dispensary. To qualify for a designated caregiver identification card, an individual must:
- Be a resident of the State of Illinois at the t ime of application and remain a resident during participation in the program;
- Serve only one registered qualifying patient;
- Be at least 21 years of age; and
- Pay an application fee.
- Persons who have been diagnosed with six months or fewer to live by a licensed physician, qualify for a free Terminal Illness Registry Card and may designate a caregiver free-of-charge.
- A qualifying patient under 18 years of age may identify two designated caregivers. There is no fee for the first designated caregiver.
Can I use an out-of-state medical card in Illinois?
Illinois does not accept out-of-state medical cards.
How long will it take to get my medical card in Illinois?
- As soon as a completed application is received, the state of Illinois will issue provisional medical card. It usually comes within half an hour or so!
- A ‘permanent’ card is usually issued within a month.
- Both cards are digital, so most patients keep a picture of it on their phone or print it out and laminate it.

How often do I need to renew my medical card in Illinois?
In Illinois, medical cards validity depends on whether a qualifying patient or a caregiver has a one-year, two-year, or three-year validity registry ID. Illinois medical cards are renewable upon expiration. At the end of each term, you will be required to renew your registry identification card and submit new documentation.
Is my Illinois medical card valid in other states?
