WHO

Provides personalized support to Service Members, Veterans, caregivers and their families who may be facing challenging hurdles by getting them the services and resources they need and have earned.

WHO WE HELP

If you fit into any of the following criteria, then and your family are eligible for services from Code of Support Foundation.

  • Current active duty, Guard/Reserve

  • Veteran who served at least 180+ days of active service (post-training)*

  • Immediate family member of a US service member or veteran

  • Caregiver to a US service member or veteran

  • Surviving Family Member

*Exception: Discharged from the military because of a disability (not pre-existing), early out or hardship

Our Case Coordination Program: Two Specialized Roles, One Mission of Support

Code of Support Foundation’s Case Coordination program is structured to provide both comprehensive and immediate assistance to veterans, service members, and their families.

We’ve expanded the program into two specialized roles: Case Managers and Community Resource Navigators. This structure allows us to better meet the complex and diverse needs of those we serve.

  • Case Managers – offer intensive, long-term support through personalized care planning, coordinated services, and regular follow-ups.

  • Community Resource Navigators – provide short-term guidance to address immediate needs and help veterans access local and federal resources quickly.

Together, these roles ensure every client receives the right level of support at the right time.

CASE MANAGERS CAN HELP WITH

Community Resource Navigator CAN HELP WITH

  • Assists veterans with cognitive, physical, and emotional deficits by providing targeted interventions and service.

  • Identifies and connects families with external support systems when caregiving needs exceed the family’s capacity, ensuring the veteran receives appropriate and sustained care.

  • Meets one on one with veterans and/or family members to assess their emotional, physical, and mental wellness to determine the gaps in care

  • Works with the veteran and/or family members to establish a care plan.

  • Researches, facilitates, and evaluates options and resources to determine the best programs that fit with the veteran’s short- and long-term goals.

  • Advocates and works alongside healthcare providers, social services, and nonprofits to coordinate care and services for the veteran.

  • Checks in regularly with the veteran at scheduled times to ensure continuity of care.

  • Meets with veterans to gather information about immediate needs.

  • Navigates available resources to include social services, VA services, and nonprofits that enhance the physical, mental, and financial wellbeing of the veteran and their family.

  • Assists with obtaining the resources for which the veteran qualifies to meet the short-term immediate needs.

  • Follows up with community programs to ensure that the veterans and their families received the services.

  • Develops and maintains relationships with community resources to easily connect veterans to available programs.

  • Once immediate needs of the veterans and their families are met, conduct 3-,6-,and 9-month follow-up.

REQUIRED DOCUMENTATION

In order for us to provide support, we will require the following documentation. Please have this ready when you reach out.
  • Service Records (DD-214, DD-215, or ERB)

  • Disability decision (PEB, VA Rating Letter, etc.) if applicable

  • Government Issued Photo Identification (Drivers License, State ID, Military ID, etc.)

  • Proof of all household income (i.e. most recent LES or RAS, civilian pay, disability benefits, pensions)

  • Related documents requested to facilitate assistance

I was in a month-to-month apartment rental but when my fiancé lost his job due to COVID-19, we missed one month’s rent and got 24hr notice to move out. Even though I contacted an attorney at the VA, they told me that I had no tenant’s rights without a signed lease.
