@rawslugs, I had an older 2FA plugin installed (https://wordpress.org/plugins/two-factor-authentication/) that generated a conflict of some sorts. I just removed it, and now I rely only on the 2FA of wp-cerber.
I have the same problem, although not having any rules on Cloudflare. Every time I try to access the custom login URL, I’m locked out with the same explanation: none existing username.

You most probably have an error in your .htaccess.

Same here. After deactivating/reactivating JS and CSS minify, the error is now
File ".../public_html/wp-content/plugins/js_composer/assets/css/js_composer_front.css" doesn't exist.That file doesn’t exist in ages in that plugin. I read several articles regarding the problems W3TC can create, but I didn’t know it can crash an entire server. Until now, that is. 🙁

I know the welcome popup is old, but up until the last updates IT WORKED. Now it doesn’t. What’s so hard to understand? If I comment out the add_action in 3.10.16 it’s working, but I don’t want to make any changes to the plugin sources and loose them on the next update. So? Any idea?

As I said, it seems that the conflict first appears when adding adrotate_welcome_pointer action.

Anyway, the last working version for me was 3.10.13. I would have rate this plugin 5 stars, but every update since then messes things up badly!
