Unclear what the issue here is. It’s almost certainly not caused by this plugin.
Try leaving the “From name” and “From email” fields blank, then the plugin will have no effect on them.
You’ll need to ask your mail server provider for the settings required.

Sounds like the issue lies with the mail server rejecting your messages for some reason. Unclear why that is, but likely nothing that can be changed via this plugin. Suggest you try asking the mail server provider for more information about what changes you should make. Sorry can’t be of more help. 🙁
Perhaps your host is blocking connections. I’ve read that is an issue with GoDaddy.

Unrelated to this plugin, suggest you look into the form plugin for the answer, this plugin doesn’t modify the content of emails.
I’m not sure if exchange is supported by PHPMailer. This plugin only sets options in PHPMailer, it doesn’t handle the actual sending of emails. Suggest you research PHPMailer’s compatibility with your mail server.
What settings are you using for gmail?
Suggest you try woocommerce support, doesn’t sound like an issue in this plugin.
