Submit ticket page not working properly

    Hi dev,

    I just installed your plug-in to test out but I found out that the “Submit Ticket” page only have a subject field nothing else.

    Could you help me with this?

    Hi:

    Sorry to hear you’re having an issue. Lets start from the basics – can you give us some information about your environment?

    1. What version of WordPress are you running.
    2. What version of PHP are you using?
    3. If you turn on the wordpress debug flags, is there an error message shown in the debug.log file?
    4. If you temporarily switch your theme to the 2015/16/17 basic WordPress theme, do you see the other fields?
    5. If the answer to #4 is no, what happens if you disable the other plugins? Do the fields show up? (Please make backups before doing #4 and #5).
