Skip Menu

I am trying to enable the Skip Menu or Skip to Content Menu. Everything I have tried places it in a new navigation bar vs. it showing up when someone uses the Tab button as a blind person would do when using a screen reader. Any suggestions for making this work behind the scenes vs. showing up on the page?
