Radio and Drop downs

Hiya, Ive been struggling with this for two days so hoping someone can help. In my contact form I have the usual name and email followed by a radio selection then a dropdown selection before the main text area. Everything is displaying as it should but when the form gets filled out and submitted and I open the email; It’s missing the intake information from the radio and dropdown selections. I cant figure out what Im doing erong but no matter what I try the emails are coming in without that info. help!
