Problem with front end entries display

Using technique described on https://calderaforms.com/doc/caldera-forms-wordpress-form-entry-viewer/ as:

Expecting list of entries, showing field contents.
Instead I’m seeing: https://www.kma-web.com/test/CalderaScreenshot.jpg

I.e. entry IDs plus link to full entry via “Details”

Any hints on how I’m going wrong would be hugely appreciated!
