If you see just a ‘Blank page’, it may be a JavaScript problem.
1. No JavaScript execution allowed on your browser.
2. There are some incompatibility with JavaScripts I using with the browser you using.

I would ask you whether the results are tied to an individual user name or not – and if they can be retrieved that way.
That’s the kind of functionality I want ask for:
– Pre and Post questionnaire (4 – 5 questions each) – per unit or module.
– Each questionnaire tied to an individual user name
– Results can be individually and collectively downloadable in a report on excel sheet
