Please help achieve this

I have successfully created what I want in the backend. It looks like this.

I want the exact thing on the frontend so that medical staff can upload their documents for verification before being assigned relevant tasks. We need the documents uploaded to remain visible in the frontend just like the image that I have shared.

I have a repeater field group name – upload
A select field with a slug – document-type
A field of type upload called – the-file
And a Textarea type field called – notes

If you’ll be kind enough to assist us we’ll be very grateful for this. Very good plugin.
