Can you please share a screenshot of how you have added the Form with the Oxygen page builder? Did you use the FluentForms module in Oxygen or a shortcode module to add the Form?

I have the same problem. Before updating to Oxygen 4.0 it worked fine, now the forms don’t show up anymore, also if i place them again or try to place a new one …
This only happens using the module. If i use the shortcode it works, but i need the ability to edit the form in the module …

I can confirm this. Oxygen Widget for FluentForm is not working after latest 4.0 release by Oxygen Builder.
It does not work for previous widgets or the new ones created after update.
Shortcodes are still working.
I checked page source and what’s happening is that form is not rendering at all.
