Please go to Mailchimp for WP > Mailchimp and there click on the Renew Mailchimp lists button.

Once that is done, go to Mailchimp for WP > Form > Settings and make sure the correct list is still selected and hit save.

Finally go to Mailchimp for WP > Form > Fields and make sure all the required fields are added to the form.

    Now test the form again.

Hope that helps. If you have any questions, please let me know!

I have this same issue and I completed the steps you outlined, Lap. However when I click Save on the Form > Settings page, the page reloads and all of my selections are removed. It’s not saving the list I chose this form to connect to.

Nevermind, deleting and reinstalling the plugin seemed to resolve my issue.
