Thank you for the quick reply. I tried installing older versions and found the following:
1.2.1 (first support for multisite) – forum general options appear to all be present, no forum specific user edit options.
1.2.2 & 1.2.3 same as 1.2.1
1.2.4 – full forum general options & user edit option of notification of new topics, no moderator option (as image above)
1.2.5 same as 1.2.4
1.2.6 is where forum general options reduce to forum location only (as image above)
1.2.7 – latest same as 1.2.6
Hope this may help you. I am happy to run 1.2.5 for now.

Update: Turns out I was looking at a user with admin rights. Non-admin users do show options for moderator, banned, etc…
So I guess only issue is what broke with 1.2.6 that made the forum options go away.

Thank you for your investigation. Yes, you cant bann admin users or make them a moderator (because they are admins anyway). You also cant do the actions to your account by yourself.
Because of the other problem: Are you sure that you have selected the correct “Forum location” page which contains your shortcode? If so, does something happens when you press the “Save options” button?
When I did an upgrade from 1.2.5 the “forum location” was the only option that showed and was correct so I never hit “Save options”. I just now went and hit “Save options” and then all the other options showed up.
Sorry about all the confusion and I appreciate your timely responses and assistance. It just wasn’t obvious to me that if the forum location was correct that I needed to press save.
