Forms plugin suggestions (specific features)

Hi all,

Looking for a recommendation for a forms plugin that can handle the following requirements. Ideally a free one is best but I understand the nature of the request may venture onto “pro” versions.

I am building a messaging system that is used to forward on telephone messages to recipients, there will eventually be upto and over 1000 forms once this is rolled out and had time to expand.

Here is a list of what I need…

– Date and time field in the UK format (this is the combined date and time picker)

– Required fields

– Notes on forms (text note for the user)

– customisable email templates

– email logic / workflow – the ability to use logic in the email workflow
( if field1 = x then send email to recipient y)

– ability to call in data from wordpress code, e.g custom variables I define somewhere like functions.php or a plugin to be used in either form fields or in the email template

– Dynamic signatures (based on the point above) – ability to pull in a custom variable (need to be able to set email sigs somewhere in wp e.g. $emailsig1 $emailsig2 and be able to define which is attached to an email depending on the form.

– a front end entry viewer for each form (fairly standard these days)

– ability to schedule weekly or monthly CSV export of specific forms and email to a different address for each form.

– conditional logic on the form to show / hide fields during input

– a conditional dropdown for “recipients” like a send to field. This should be able to dynamically replace the “to” address field with a corresponding email address

So far I have been using Caldera Forms and I have most of the things set up that I need, the main issue I have with Caldera is the scheduling of CSV export via email. There does not seem to be a simple way to do this.
