Sorry to hear you are having this issue.
I can see some JavaScript errors when we access the page from a regular URL which doesn’t happen when we access without no caching:

This indicates that Cloudflare or an Optimization plugin is causing the issue, can you try to clear all caches and check your optimization plugin? Make sure to exclude the jQuery-core file from optimization to prevent some issues.

I visited the site again and I can also see those JS errors.
It seems that most of JS assets are served from yet another CDN and I’m not sure if there’s any additional optimization/caching plugin but I suppose there is.
I have some additional questions then:
1. are you hosting the site with WordPress.com or with different host but you are using Jetpack plugin?
2. If it’s different host than WordPress.com but you are using Jetpack, can you check in it’s “Performance” settings if there’s an option enabled to speed up loading of JS/CSS (or, otherwise, “static files”) and if so – disable it, clear all caches (including ClouFlare) and check site again?
3. If above is not the case or doesn’t help – what other plugins exactly are you using for caching and JS optimization on site?

Hello Adam,
My site runs on WordPress with a different host.
I am using Jetpack but there is no “speed up loading of JS/CSS” in the options. I’ve disabled all optimization apps to see what might have caused it.
Here’s a Screenshot of what I am seeing.
The forms have already started to show with Jetpack and Asset CleanUp: Page Speed Booster plug-ins activated though. I initially had Cloudflare and Asset CleanUp: Page Speed Booster but these are all disabled until I figure out what’s causing issues.
