Double-check your settings to be sure they are correct, most importantly;
– The PDF field’s EXPORT DATA value must match the Formidable Form checkbox value, exactly.
– Each PDF field Name must be unique.

Is this right? It doesn’t work this way.
That’s my understanding – if that’s not right there needs to be a video walkthrough of this if possible.
Each Formidable Form Checkbox Field (no matter how many options it has) must be mapped to each PDF Field Checkbox and each PDF Checkbox must have a unique field name.
So, if the Formidable Form checkbox ID is [100] and has 4 options (ANSWER1, ANSWER2, ANSWER3, ANSWER4)
Then, map [100] to each of the 4 PDF Checkbox fields named (ANSWER1, ANSWER2, ANSWER3, ANSWER4) each with an EXPORT VALUE to match (ANSWER1, ANSWER2, ANSWER3, ANSWER4)

FYI: There is a known issue with corrupting checkbox fields if they are incorrectly mapped. So, if all is mapped correctly, try replacing the PDF checkbox(es).
