About the recurring patients: the core theme I came up with was the transience of human life and the idea that everything loops back to the same point. When you enter the first room, you see a patient leaving the hospital happily. In the next stage, a new patient—Richard, the main character—admits himself. Patients come and go, but Richard remains. By the end, when he finally leaves, a new patient admits himself in that room—completing the loop, suggesting this cycle will continue indefinitely.
As for the riots, they don’t carry a deep narrative meaning. Their purpose was to give the player a little more to question and to extend Richard’s stay. It also serves to show that there’s life happening outside his room, adding a bit more depth to his story.
