This was not able to do the one thing which I needed it to be able to do, which was create an HTML document that preserves the PDF's original forms.

to install on macos, you can use "brew" 1) install xpdf: "brew install homebrew/x11/xpdf" 2) install pdftohtml: "brew install pdftohtml" to install on ubuntu/linux 1) apt-get install poppler-utils
