I'm pretty sure that's a Sturmpanzer Brummbar for use in urban areas, a sort of heavy assault gun. It was mounted on a Panzer IV chassis and mounted a 15 cm howitzer for direct infantry support. They only built a relative handful of them, and they were used on all fronts. By the end of the war, most had either been destroyed in action or blown up by their crews to keep the enemy from capturing them.

It looks right, and Tamiya has a model of at least one version. There could easily be others models out there, or they could easily modify the Tamiya to look like this..
