This app has created to help hospitals educate and monitor patients in order to prevent hospital readmission, reduce morbidity and mortality, improve patient satisfaction and aid in reducing healthcare costs. Following congenital heart surgery, patients and families need diligent transition from hospital to home care. In the past education was done face to face with paper instructions given. This discharge app changes historical discharge teaching by offering a mobile platform to give instructions along with the robust features of a mobile device. It offers in app reminders for taking medications, the ability to provide home monitoring for high risk populations, phone list with in app dialing. This app was designed as a Doctoral Project to improve health care technologies in order to improve care to families. The author of the app is in his final year of the Doctor of Nurse Practice program at the University of Florida College of Nursing.
Requires iOS 6.0 or later. Compatible with iPhone, iPad, and iPod touch.
