In-School Credit Program Application

Complete the form below to apply for the In-School Credit Program. Before submitting, please make sure you have the following information ready:

1. Student information (name, email)
2. Parent/guardian email — required, as a parent signature and review is needed to complete the application
3. High school and counselor information — if you don’t know your counselor’s name, select Unsure in the counselor dropdown

Once submitted, your parent/guardian will receive an email from SignNow to review and sign the application. After the document is complete, it will be automatically forwarded to your counselor for processing.
