The National Collaborating Centre for Determinants of Health (NCCDH) works to move knowledge into action on the conditions of daily life in which people are born, grow up, live, work, play, learn, and age and the driving forces that shape these conditions – the structural and social determinants of health.
NCCDH supports uptake and integration of relevant knowledge and evidence in public health practice, programs and policy through development of evidence-informed resources, exchanging promising practices with the Canadian public health community and bringing together diverse voices and multisectoral perspectives.

Opportunities and challenges for moving upstream: Summary of a researcher dialogue to explore how research can contribute to understanding and acting on the structural and social determinants of health
This resource shares results of a dialogue convened with population and public health and social science researchers to explore how research can contribute to understanding and acting on the structural and social determinants of health. The overarching message from participants is the need for transformative change in the way public health research is done.
Let’s Talk: Moving upstream
This Let’s Talk resource uses a stream analogy to explore different levels of public health action to advance health equity. Through descriptions of upstream, midstream and downstream approaches and practice examples, it encourages public health practitioners, decision-makers and policy-makers to work against the current and shift efforts upstream to address the structural and social determinants of health.
Using health equity frameworks to advance health equity in organizational and system contexts: Learnings from the field
This workshop report presents considerations and guidance on ways to use health equity frameworks to disrupt systems of oppression and advance health equity. The report is grounded in the practical knowledge, expertise and experiences of approximately 50 workshop participants from public health, health care and social service organizations.

This practice brief describes four public health roles to support community-organizing efforts and advance health equity. Actionable steps and practice examples are outlined for each role to guide its implementation.
Toward a safe and healthy place to call home
This document summarizes results from a 2023–2024 survey of Ontario public health unit practices to promote healthy housing. Highlights of survey results are presented alongside public health roles and promising practices to address inadequate and unhealthy housing conditions as a determinant of health and key driver of health inequity.

Immigration status is a significant yet often overlooked determinant of health. This issue brief outlines the health and health equity impacts of immigration status for migrants in Canada to build understanding and guide public health planning, policy and action.
