In 2018, Adrian McCullough, our founder, was the main caregiver for her father who was fighting chronic medical issues. Adrian was quickly frustrated as all of the home health agencies that she hired had constantly changing aides, last-minute call-outs, and sedentary caregivers. Courageous and energetic, Adrian told her husband since there wasn’t a home health agency that delivered consistent & personal care that she was going to start her own home health agency. And that is exactly what she did! Today Cornerstone has steadily grown into a reliable & trustworthy home health agency.
Unfortunately, Adrian was diagnosed with an aggressive cancer and she sadly lost her fight in March of 2020. It is very important to us at Cornerstone that Adrian’s name and purpose are passed on to our clients. Cornerstone’s strong intention is to continue Adrian’s legacy of a small personalized home health agency that lovingly treats you like family.
Cornerstone Health Partners was started by Adrian McCullough after finding the need for specialized assisted care for loved ones of her own.
Adrian was born and raised in Wilmington, Delaware but spent many of her summers at our Delaware beaches with family and friends. She graduated from Dickinson High School in 1993 and later, after raising three children, she went on to finish her education at Wilmington University, where she received two masters degrees in Business and Health Administration. She spent many years in the pharmaceutical industry where she ultimately discovered that her passion was for the people. Adrian was a primary caretaker for her father when this passion first sparked, leading her to launch her ever-growing home healthcare business, Cornerstone Health Partners in 2018. The success of CHP had always been Adi’s dream and will continue to be her legacy.
Anne Weaver earned her Bachelor of Science in Developmental Psychology from Emmanuel College in Boston, MA, in 1992, followed by a Master of Science in Counseling from Wilmington University in 1996. Her career began with eight years of dedicated counseling service within the Diocese of Wilmington, where she was a crucial member of the Crisis Team for Wilmington Schools. Here, Anne provided vital support to children and their families, facilitated community-funded programs, and led Grief and Loss Groups in collaboration with Catholic Charities and the Red Cross.
At the inception of Cornerstone Health Partners, Anne came on as the Vice President in November 2018, Anne has played a pivotal role in shaping the company’s strategic direction. Under her leadership, Cornerstone has enhanced its service offerings, focusing on integrated health solutions and expanding its outreach to meet diverse community needs. Anne’s expertise in counseling and developmental psychology significantly informs her approach to leadership, emphasizing empathy, patient-centered care, and innovative health programs. Her vision is to continue driving Cornerstone towards excellence in healthcare services, making a significant impact on the lives of patients and their families.
Beyond her direct counseling work, Anne is a committed board member of the Beat the Beast Foundation, a non-profit that assists individuals undergoing cancer treatment, reflecting her deep commitment to health and well-being.

Molly Hayes graduated with a Bachelor of Science in Nursing, with Distinction, from the University of Delaware in 1994. She began her career in healthcare as a Neurosurgical ICU nurse at the University of Virginia Medical Center, where she dedicated eight years to caring for critically ill patients. Molly then transitioned to the Preanesthesia Admissions Testing Unit at UVA, spending two years working alongside anesthesiologists to evaluate high-risk patients preoperatively. Following her hospital roles, Molly served as a school nurse for six years under the Diocese of Philadelphia, further diversifying her nursing experience. Since 2018, Molly has held the position of Clinical Director at Cornerstone Health Partners, managing client care and supporting the clinical staff.She lives in Wilmington, DE, with her husband, three children, and two dogs.
Samuel Weaver is the Director of Operations at Cornerstone Health Partners, where he applies a decade of experience in property management to enhance healthcare operations. He earned a bachelor’s degree in Business Administration from Coastal Carolina University, equipping him with a robust foundation in strategic planning for Cornerstone’s future! Samuel’s commitment to professional development is demonstrated by his recent participation in an emerging entrepreneurs workshop at the New Castle Chamber of Commerce. In his role, he focuses on improving operational efficiencies and ensuring the organization adapts to the changing needs of the healthcare community. Outside of his professional life, Samuel is blessed with a unique perspective brought by being the middle sibling in the Weaver family. His work at Cornerstone continues to foster a dynamic environment where both the staff and the services they provide can thrive.
Ty Weaver has been a dynamic presence in the automotive industry since his tenure at Enterprise Rent-A-Car began in 2016. His career advanced significantly when he earned his Commercial Driver’s License (CDL), which enabled him to take the helm at Cornerstone Cars.At Cornerstone, Ty oversees the transportation operations with a keen focus on safety and customer satisfaction, ensuring our services meet the highest standards. His commitment not only reflects his professional dedication but also his personal investment as a member of the Weaver family and a father to a son, enriching his understanding of the diverse needs within our community. Ty’s leadership continues to propel the growth of our transportation services, supporting the mobility and independence of those we serve at Cornerstone Health Partners.
Cindy Beach earned her bachelor’s degree in nursing from Northern Illinois University in 1982. After relocating to Wilmington, Delaware in 1996 with her husband, Ken, she dedicated her career to home health care, serving communities in both Illinois and Delaware. In addition to her professional commitments, Cindy is passionate about traveling, playing tennis, reading, and enjoying time at Dewey Beach. Cindy joined Cornerstone Health Partners in October 2020, bringing extensive experience and a heartfelt dedication to her role as Director of HR. She leverages her nursing background to enhance employee relations and workplace wellbeing. Cindy is a proud mother of two adult children, emphasizing a balance between her professional achievements and family life.
Jessica Weaver, a valued member of the Weaver family, holds a Bachelor of Liberal Arts from Wilmington University, initially pursuing Fine Arts at the University of New Haven. Her diverse background, combining art and design with comprehensive financial and communications expertise, significantly enhances her role at Cornerstone Health Partners. With over eight years of caregiving experience, Jessica specializes in companion and end-of-life care, providing sensitive and expert support to a diverse population. She has been a dedicated part of the Cornerstone team for three years, where her creative insights are instrumental in developing innovative care strategies. Outside of her professional life, Jessica is also a devoted mother to a three-year-old, which enriches her perspective on family and community care. Jessica’s multifaceted skills and personal dedication are integral to Cornerstone’s mission to deliver empathetic and comprehensive community care.
