Vision Statement
The vision of MyCHN is to be a leader in providing quality services and programs that enhance the lives of the people in our community.
Values
The values of the Center are quality care at reasonable costs, education to promote healthy living, access to needed care, transparency in all aspects of our operations, competent and empathetic staff, high professional standards and commitment to advances in healthcare.
Our History
Formed in April 2008, MyCHN replaced a Federally Qualified Health Center that had just lost its funding. This meant a fresh start and a purpose-driven beginning for MyCHN and the Alvin community. We started in Alvin, Texas, with one location, one provider, and three exam rooms. Starting small not only allowed us to use our imagination and dream big, but it also allowed us to think about what we want to be and how we want to change the healthcare the communities we serve receive.
Our Purpose -
Patients & The Community
There are people in need of quality healthcare, but sometimes they sacrifice healthcare for more prevalent necessities. MyCHN understands that community members put food on their tables, pay their bills, and care for their families before paying for any healthcare services. This leaves a high number of families and individuals without any affordable access to care. We want to give them the opportunity to do all of this while being healthy. So, that became our purpose and drive. Affordability, accessibility, and quality became our focus for the communities we serve.
MyCHN Empathy Award Program
At MyCHN, we love to recognize our employees in a different way. They work hard to positively change the communities and patients that we serve, and because of that, they deserve to be recognized.  The Empathy Award is our recognition and gratitude program for exceptional and unique, heartfelt heroes at MyCHN.Â

Affordable Care for All
MyCHN accepts most private insurance, Medicaid, Medicare, and CHIP. Uninsured individuals may be eligible for discounted self-pay services based on their family size and household income. MyCHN strives to provide the highest value of care and therefore offers substantial benefits for people who are uninsured and self-pay.Â
We have controlled the cost of office visits, and we have kept lab and pharmacy costs far below other providers. Low-income, uninsured patients may apply for discounts to the cost of their care. MyCHN does not turn away anyone away due to an inability to pay.
FTCA Deeming
Stephen F. Austin Community Health Center, Inc., dba Community Health Network (MyCHN), receives HHS funding and has Federal Public Health Service (PHS) deemed status through the Federal Tort Claims Act (FTCA).
As such, MyCHN is granted medical malpractice liability protection through the federal government acting as its primary insurer. As a deemed health center, MyCHN and its employees, working within the approved scope of project are considered federal employees and are immune from lawsuits for medical malpractice.Â
A patient who alleges acts of medical malpractice by a deemed health center cannot sue the health center or the provider(s) directly, but must file the claim against the United States Government.
