The Women, Infants and Children program at ACC, better known as WIC, is one of the cornerstones of our vision to provide basic human needs to all. Our program focus is to meet the nutritional needs of expectant and postpartum mothers and for children up to age five. The WIC program demonstrates several positive outcomes.

WIC SPECIAL FORMULA/FOOD REQUEST FORM
The Special Formula/Food Request must be completed by a health care provider licensed to write prescriptions under state law, then provided to the WIC clinic as an original written document, electronically or via facsimile.

Nondiscrimination Statement

To file a program discrimination complaint, complete the USDA Program Discrimination Complaint Form, AD-3027, found online at How to File a Program Discrimination Complaint and at any USDA office or write a letter addressed to USDA and provide in the letter all of the information requested in the form. To request a copy of the complaint form, call (866) 632-9992.
