Annually, IFIC organises the International Conference on Integrated Care (ICIC), the largest conference of its kind and ideal platform for showcasing projects and engaging directly with the integrated care community.

UNICA4EU

UNICA4EU aims at helping children and adolescent with cancer through AI: benchmarking algorithms for early detection, diagnosis, treatment, risk scores and patient stratification to build a framework that can improve the stand of care and address unmet needs for patients, caregivers and clinicians.

EU4CHILD

EU4CHILD is meant to lay the foundations for the development of a multi-purpose crowdsourcing ecosystem for Pediatric Cancer Genomic Data inventory, able to support a set of AI-based services for several types of cancer detection, diagnosis, prognosis and treatment.

INCAREHEART

Five INCAREHEART public procurers from five countries will jointly procure an ICT-enabled integrated care solution to effectively support the management of a multidisciplinary care and support model for people living with Chronic Heart Failure (CHF).

CAREMATRIX

Pre-commercial procurement for integrated CARE solutions addressing the Multimorbidity mATRIX in ageing populations. Over 50m people suffer from multimorbidity (PMM) in Europe. Recent reviews find that PMM experience a range of system and professional-related issues with care delivery.

Project ValueCare

Project ValueCare is an EU funded project which aims to deliver personalised integrated (health and social) care services, better outcomes for older people and improved care experience. It also aims to improve staff satisfaction and greater efficiency in the use of resources and coordination of care.

PROCare4Life

PROCare4Life is an EU funded project proposing an integrated scalable and interactive care ecosystem, easily adapted to several chronic diseases, care institutions and end-user needs, benefiting patients, caregivers and health professionals.

VIGOUR

VIGOUR effectively supports care authorities in progressing the transformation of their health and care systems to provide sustainable models for integrated care which will facilitate identification of good practice and scaling-up.

EURIPHI

Introducing innovation and integrated care solutions in Europe’s health and social care systems through cross-border value-based innovation procurement.

DigitalHealthEurope

DigitalHealthEurope will provide comprehensive, integrated and centralised support to the Digital Health and Care Innovation initiative in the context of the Digital Single Market Strategy.

TransForm

The focus of TransForm goes beyond the most common interpretations placed on integrated care. Rather, TransForm seeks to examine how co-productive partnerships that empower people in local communities can be developed through cross-sectoral collaborations.

CANCERLESS

The aim of the CANCERLESS project is to deliver an innovative solution as an aggregate intervention based on the combination of the tested Patient Navigator Model and Patient Empowerment Model to create the Health Navigator Model for Europe.

ROSIA

ROSIA (Remote Rehabilitation Service for Isolated Areas) proposes to generate a flexible and scalable value-based model of care, organized around self-management, or self-care of rehabilitation at home, designed from a tailored integrated care model which optimizes the quality of care and the use of clinical resources.

Inter-American Development Bank

Inter-American Development Bank work to improve lives in Latin America and the Caribbean. Through financial and technical support for countries working to reduce poverty and inequality.

SUSTAIN

SUSTAIN is a cross-European research project and stands for sustainable tailored integrated care for older people in Europe.

IMPLEMENT – accelerating improvements in care and support for people with chronic illnesses across the European Union.

United4Health

United4Health develops telehealth solutions that enable healthcare providers to adopt innovative and integrated health and care service models.

CAREWELL

CAREWELL is a project that is examining technological solutions to support multi-level integration for patients with complex needs through ICT.
