Case Management Overview
From food stamps to housing vouchers and employment hurdles, people experiencing housing instability are often navigating a complex web of policies and procedures to fulfill their basic needs. For this reason, we offer one-on-one housing case management services for individuals enrolled in specific Housing Assistance programs. We assist these clients in formulating a plan to stabilize their current housing and utilize housing resources.
Case management services are limited to select programs at Housing Assistance. Case managers can provide valuable support in the areas of housing search, housing application, navigation of local resources/providers, and guidance on all aspects related to clients remaining stably housed. They can also help break down the many aspects of housing governance and policy that can, at first glance, appear challenging or overwhelming amid times of housing instability.
Falmouth Case Management
Residents who live in Falmouth and are currently experiencing housing instability could receive a referral to a Housing Assistance case manager to address their housing-related needs, thanks to a partnership with Falmouth Human Services. These needs include but are not limited to:
- Eviction concerns
- Rent arrears/owed utility payments
- Housing applications
- Adverse housing situations
- Currently residing in a temporary and/or unstable housing situation
Youth & Young Adult (YYA) Overview
The Youth & Young Adult (YYA) program provides individualized support for unaccompanied youth ages 16 to 24 on Cape Cod who are experiencing homelessness or housing instability. Through case management, temporary and permanent housing solutions, and connections to essential resources like education, employment, and mental health care, the program empowers young people to achieve stability and independence.
