  • keep hook nudge fail-open so SessionStart never blocks on nudge issues
  • log warnings when soul/logs state is unreadable or malformed
  • add explicit code comments documenting the fail-open design intent
  • add test coverage for malformed soul behavior in nudge
  • update SPEC.md to document fail-open nudge error handling
